reventive Maintenance Requirements

First time customers must call Uptime Medical at (704) 454-3264 to set up an account and customer number.

The following information must be provided with each order.

• Your customer number
• Your facility name
• Contact name and phone number
• Detailed problem description
• Purchase order number
• If a warranty return, you must provide a return authorization number obtained by calling Uptime Medical.

* PM service does NOT offer any repairs of any kind. If device fails testing, the customer will be notified and offered Repair or Exchange service. Normal PM rate applies for customers requesting units sent back not repaired. See terms and conditions.
